My Medical Records
ORMC values your privacy rights and takes many precautions to safeguard your health information. We want the right people to get information necessary for your care, and make your access to information as convenient as possible.
Your medical record contains documentation by all physicians and healthcare providers involved in your care and treatment while you were here at ORMC. It contains the results of diagnostic tests and therapeutic procedures performed, as well as information about your medical history, and your condition and progress during your hospitalization.
How Do I Receive My Medical Record Information?
Federal HIPAA laws require an authorization to be completed for all medical record requests, unless your provider is requesting them for your care, in which case a delay to have a sick or injured patient complete a form is unreasonable. If you are requesting records for personal reasons, please complete the request form.
Your medical record, and the information it contains, is confidential. Therefore, we must confirm your identity before releasing your medical record. You can download the authorization form here. Only the patient or his/her legal guardian can sign the form. The form must be returned with a copy of a government-issued picture identification.

You will receive an invoice, and you should receive your records 5-7 business days following the invoice.
If you are unable to print the form from this website, you may pick up a paper copy of the authorization form in person. Please visit the Information Desk in the front lobby of the hospital. Packets are available which include the form and instructions. You may fill out the form and turn it back in at the desk. The information desk will forward to the Release of Information specialists for processing.
If your records are needed urgently for an appointment or other patient care, please have your provider contact the Medical Records Department to expedite processing of the request. Courtesy copies can be sent to your provider in a quicker time frame than if requested by the patient for personal reasons.
